Each floor has it's own boss that needs to be defeated before the next floor can be access. The boss only has to be defeated once by any player or team. Any previously cleared floor can be travels back to.

Small things like in this episode, Kirito doesn't actually kill Rosalia and they do revive the pet, and then full story changes in some of the later episodes. (Which changed some of the more... Uncomfortable parts of the Anime) Edit: Also one thing that might be useful to keep in mind, Kirito is about 15 years old (at this point) and is over leveled making him basically a god to other players. So his morality (and general life experience) in this video game, with adults trying to kill him, might be a bit off.
